Rehabilitation: Individualized therapies of physical, occupational and speech

Our post-acute rehab programs feature individualized therapy programs and include physical, occupational and speech therapy. When you or a loved one requires a higher and more regular level of care, our skilled nursing is there to help. We offer rehabilitative care, restorative therapy and long-term care in our on-site skilled nursing center. We subscribe to a philosophy of personalized care, which emphasizes each person’s quality of life, dignity and comfort. Residents are in control of their days, choosing which services they want and when they wish for them to be delivered. We believe the best way to get someone back on their feet, faster, is to offer them a calm and soothing setting where they can simply focus on healing. In addition to physical, occupational and speech-language therapies, our therapy services address memory issues, vision issues, incontinence and recovery after stroke.

Post-acute rehabilitation services are for people of all ages who no longer need hospital care, but would benefit from additional therapy and care before returning home following injury, illness or surgery.

Admission Requirements
Admission to the health services center requires a thorough assessment and consultation with our professional team and the individual’s personal physician. We invite you to discuss your particular needs with us by calling the Admissions Coordinator.

Long-term Care
Residents receive high-quality 24/7 care from our trained and compassionate team members, who are dedicated to providing loving, family-like care. Staff is consistently assigned to the same residents, which helps strengthen the care bond and fosters close relationships and better outcomes.

To effectively manage each resident’s complex health care needs, our staff works together, and with the resident’s personal physician, to create a comprehensive yet flexible plan for care that addresses the resident’s unique needs.
